Once completed, we will provide you with the sealed envelope containing Form I-693 to present to USCIS.
Citizenship Physical Mini Mental (Form N-648)
For individuals applying for U.S. citizenship who have a medical disability preventing them from fulfilling the English and civics requirements, the N-648 form is essential. This is often referred to as a “Medical Certification for Disability Exceptions.”
